I use a bug reporting system at work where engineers can make comments, assign actions, and so forth.
For important bugs, multiple engineers often try to update at the same time. When updates conflict, the first person in gets accepted and the second person has to try again (comments are saved, at least). Sometimes I have to try 3-4 times before mine are taken because everybody is updating at the same time.
